Medical

Medical coverage offers health care protection for you and your family. HOPE Program offers regular full-time employees options from ten medical plan options, provided through OneDigital. You may visit any medical provider you choose, but in-network providers offer the highest level of benefits and lower out-of-pocket costs.
Network providers charge reduced, contracted fees instead of their typical fees. Providers outside the plan's network set their own rates, so you may be responsible for the difference if a provider's fees are above the Reasonable and Customary (R&C) limits.

Dental

Dental coverage helps you maintain good dental health. The plan covers preventive care, including regular checkups, fillings, and other dental procedures. You may visit any dentist you choose, but you'll receive a higher level of benefits when you go to an in-network dentist.
Coverage is provided through Ameritas or Smile Saver. With an out-of-network provider, you may be subject to Reasonable and Customary charges not covered by the HOPE Program Plan, resulting in additional expenses.

Vision

Annual vision exams not only help keep your vision healthy, but they can also identify certain medical conditions, such as diabetes or high cholesterol. HOPE Program offers vision coverage through EyeMed Vision. Under the vision plan, you may use the eye care professional of your choice. However, when you use a participating network provider, you receive higher levels of benefits.
